When you first arrive, we ask that you fill out new patient forms on the dedicated tablet located at the front desk. Here, you can ask the front desk any questions you might have prior to seeing one of our dentists. Soon after, you’ll be greeted by the doctor directly, where they will introduce themselves and bring you to one of our operatories. Then, you’ll be introduced to one of our dental assistants and given an opportunity to ask questions or address any concerns you may have about your oral health.
Before our dentists can see you, he needs to get to know you a bit better. This way, they can fully tailor your dental experience to you. For this reason, you will need to either electronically fill out the form below or make sure to show up at least 15 minutes ahead of your scheduled appointment in order to fill out new patient forms through our dedicated tablet at the front desk. These forms will ask questions regarding your current oral health concerns, any pre-existing conditions we should know about, and insurance information if applicable.

If you don’t currently use dental insurance but wish you could get your most essential services completed at an affordable price, you’ll find our in-house membership plan to be quite helpful. For one flat annual rate, you can access exams, cleanings, X-rays, and other services at discounted prices by signing up for a plan at our dental office, either for yourself or your entire family! It’s the perfect alternative to dental insurance, which can often be too expensive or come with pitfalls you’d rather avoid.
With a dental insurance plan, paying for your most essential preventive treatments becomes even easier. If you’re using traditional dental insurance benefits to cover your dental care, we’re happy to accept a wide variety of plans at our dental office.
